Documentary Requirements for Application

- Bank Certification
- Original Passport with valid Tourist/Balikbayan Visa in the Philippines
- 8 Pieces 2×2 Photos
Requirements for Additional Dependents
Applicants intending to include their spouses and dependent children in the application must additionally provide the following requirements:
- Completed SRRV Application Form
- Completed Application Form for Dependents
- Original PRA Medical Certificate
- Original Copy of Police Clearance (and NBI Clearance, if applicable) for the spouse, as well as for children aged 18 years and above
- Apostilled or Authenticated Document as Proof of Relationship
- An application fee of US$300.00 for each accompanying dependent.
- Original passport with a valid tourist visa for the Philippines
