Documentary Requirements for Application

The following outlines the necessary documentary requirements and associated fees that are to be submitted and paid to the PRA by the principal applicant as part of the application process:

  1. Completed SRRV Application Form
  2. Original Copy of Medical Certificate
  3. Police Clearance
  4. NBI Clearance
  5. Bank Certification
  6. Original Passport with valid Tourist/Balikbayan Visa in the Philippines
  7. 8 Pieces 2×2 Photos

Requirements for Additional Dependents

Applicants intending to include their spouses and dependent children in the application must additionally provide the following requirements:

  1. Completed SRRV Application Form
  2. Completed Application Form for Dependents
  3. Original PRA Medical Certificate
  4. Original Copy of Police Clearance (and NBI Clearance, if applicable) for the spouse, as well as for children aged 18 years and above
  5. Apostilled or Authenticated Document as Proof of Relationship
  6. An application fee of US$300.00 for each accompanying dependent.
  7. Original passport with a valid tourist visa for the Philippines
